How to Fill Out the Elevate Pay Source of Funds Sheet for Your Business Account
Before we can approve your Business Account, we need to understand where your money comes from. The Source of Funds sheet lets you share that information in a structured way. Here's how to fill it out, tab by tab.

The two tabs
Overview: legal business name, business email, how you plan to use the account, and any parent company, subsidiary, or affiliated entity.
Source of Funds: exactly how money comes into your business. This tab is split into categories by income type. Both tabs are required.
Fill only one category
In the Source of Funds tab, find the category that matches your main source of income and fill only that one:
Direct Client — you invoice a company directly
Amazon — you sell on Amazon
Fiverr — company account
Shopify — your own store
Etsy — your own shop
Stripe, Square, Paddle, or Lemon Squeezy — a card processor
Google or Meta — ad revenue or monetisation payouts
Upwork — company account
Other — any platform not listed above. Name the platform in the Comments column and upload an earnings report or payout statement from it, under your business name.
If you're paid more than one way, fill only the category that covers most of your income and leave the rest blank. You can still receive from other sources.
How each category works

Example: the Direct Client category's header and EXAMPLE row.
A grey EXAMPLE row is a completed row. Use it as a template, don't edit it.
The row below it is yours to fill in. Same columns as the example. For the monthly amount, enter what you receive from that source in USD. If it varies, use the last months..
The Uploaded File Name(s) column: type the exact name of each file you upload in the app for that row, for example "abc_contract.pdf ; abc_bank_statement.pdf".
Use the Comments column for context. "Monthly retainer," "FBA store," whatever's relevant.
The ★ line: the exact documents to upload in the app with this sheet. For example: a signed contract, a bank statement, a platform earnings report. Skip it and your approval stalls.
What gets sent back
Documents go under your business name, not an individual's. Even as sole owner, a personal invoice or bank statement won't count.
Documents that don't match the category you filled. If you filled Upwork, we need an Upwork earnings certificate, not a client invoice.
Last step
Upload the completed sheet in the app as an xlsx file, along with the supporting documents for your category.
What happens next
We review your submission within 10 business days. If anything is missing, we'll tell you exactly what, so you won't need to start over.
